Consent to Groom & General Liability Terms

 

Because we care about the safety and wellbeing of all pets, we want to assure you that we will make every effort to make your pet's grooming visit as pleasant as possible.

 

By booking with us in any way (website, phone call, text message, social media, etc.), your representation to us includes the following:

You represent and warrant to us that you are the pet’s legal owner and are responsible for all fees incurred at Studio Dog for the Services performed. On your own behalf, on behalf of any co-owners of the pet, and anyone you allow to bring in or pick up your pet, you also continually represent to us each time you elect to have a Service performed on your pet.

 

  1. If during an appointment it becomes apparent that your pet is not able to be groomed due to aggression/behavior or any other issue, the customer will only be charged for what services have been performed. Minor injuries such as nicks from clippers, scissors, or toenail trimmers may result if your pet does not respond to the groomer and remain still during the grooming or toenail trimming procedures. We give our absolute best effort to try and avoid this.

  2. If your pet has any allergies or medical conditions (epilepsy/seizures, diabetes,  arthritis, separation anxiety, asthma, etc.), it is extremely important that you make the groomer/Studio Dog aware of any medical & behavioral issues so that we can take the necessary steps to ensure your pet's utmost safety and comfort.

  3. Pet guardian/Customer assumes all liabilities, financial and otherwise, for the behavior and health of their pet in the professional groomer. In consideration of the services rendered by Studio Dog, Customer waives any and all claims, actions, or demands of any nature, foreseen or unforeseen relating to the care, control, and health of customer's pet arising during or after services performed by the Groomer/Studio Dog.

  4. Occasionally, grooming can aggravate a current, or expose a preexisting medical condition. Such medical conditions could arise during or after the grooming procedure. In the event of this situation, The pet Guardian/Customer shall not hold the Groomer/Studio Dog liable. Pet guardian/Customer shall not hold the Groomer/Studio Dog liable for any skin irritations that may result from grooming procedures or any stressful effects that grooming may have on young, geriatric, timid, or temperamental pets.

  5. Pet guardian/Customer also shall not hold the Groomer/Studio Dog liable for any problems uncovered on a badly matted or otherwise neglected coat or integument. 

 

Paying for Services

You agree to pay us for the Services we provide to your pet(s) upon completion of the Services. Prices are subject to change without notice, and seasonal rates may apply. Salon pricing may vary depending on the breed, the condition of the coat, and the selection of additional Services at the salon.

 

No-Shows & Cancellations

No-shows and last-minute cancellations are subject to a $25 no-show charge fee (per pet), which may be added to your next ticket. Prepayment for some fees will be required before another appointment is booked. We understand there may be emergency situations and will work with you, but not on a continued basis. Please be respectful of our time, as we are a by-appointment business, and another client could have taken your appointment if we had known.

NOTE: Clients who are 15 minutes late risk losing their appointment without notice. Studio Dog may refuse clients that have a history of late cancellations and/or no shows. If a payment is declined, the client will be required to clear any outstanding balances before being able to book another appointment.

 

Behavior or special handling fees

In the case that your pet requires extensive time due to behavioral issues or special needs, a handling fee may be applied. The client will be solely responsible and will indemnify Studio Dog, for all damages caused by the pet’s behavior.


 

Late Pick-Ups

We do not have a boarding facility. If your pet(s) are not picked up by the end of your appointment, a late pick-up fee of $20 will apply. We provide a 30-minute grace period from the moment we call or text starting your dog is ready for picked up to when you arrive.

 

Vaccination Policy

Studio Dog enforces strict vaccination and health requirements. The number of vaccination booster requirements for vaccinations can vary based on the age of the pet and state or local regulations. Vaccinations must be administered at least 48 hours before arrival under the guidance of a licensed veterinarian. By signing this agreement, owners verify their pet(s) is current on Rabies, Bordetella, Distemper, Parvo-Virus, and other vaccinations. Proof of Vaccination or current Titer testing shall be provided to Studio Dog upon request, as well as current Veterinarian information. Special circumstances will be taken into consideration.

 

Safety Warnings

We want to make our stores safe for our pet parents, their employees, and pets. This means:

i. Contagious Diseases, Aggressive or Dangerous Pets

Animals bite: regretfully, many of our associates are in the bite zone while performing the Services. Owners MUST inform Studio Dog if their pet(s) bites, has bitten, or is aggressive to people, other pets, or specific grooming procedures. If we feel it is necessary to do so for the safety of your pet, other pets, or our associates, we may use a muzzle on your pet or refuse/discontinue the Services. Muzzling will not harm your pet and protects both the pet and the groomer. Studio Dog reserves the right to refuse/stop Services for such pet(s) at any time before or during the grooming process. The client understands that they will be held liable for all bites/injuries caused by their pet.

 

Emergencies

i. Veterinarian Authorization-Medical Emergencies

In the best interest of your pet(s), this agreement will give Studio Dog permission to obtain immediate veterinary treatment for your pet should it be deemed necessary by Studio Dog. We will do our best to contact you first, and then take your pet to the nearest available veterinarian. You agree to cover all expenses for such veterinary care upon signing this Agreement.

ii. Natural disaster

Every effort will be made to contact you or your agent to retrieve your pet in an emergency or natural disaster. Studio Dog, at its sole discretion, is authorized to transport and/or to make temporary alternative arrangements to house and care for your pet until you or your agent can retrieve it. You understand it may not always be possible to evacuate your pet safely and that, while our associates are dedicated to the animals in their care, human safety must come first in a truly critical situation. That could mean in rare circumstances, that your pet must be left behind until an evacuation, if possible, can be safely accomplished.

 

Visual Release and Use

All images, photos, and videos of the pet(s), taken during the grooming, as well as their names, can be used by Studio Dog in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials, and are the property of Studio Dog.

 

Data Collected Through Forms; Incidents

Studio Dog generally will retain only the necessary information to complete a grooming appointment. You understand and consent to the use of your information for consumer marketing purposes we may conduct. In addition, if your pet is suspected of having injured another person or animal, or you or your pet are involved in or witness an incident in our studio, we reserve the right to give your name and contact information to animal control, to a treating physician or veterinarian, to the police, to health officials and other government agents, to insurance agents, or to others reasonably requesting the information with a need to know.

 

Release

Except for the limited warranty obligations of Studio Dog described above, you release and agree to hold harmless Studio Dog from and against any and all damages, expenses, claims and other liabilities, including damages and claims arising out of or relating to the Services provided or your or your pet(s) interaction in connection with Studio Dog, that you may now have, hereafter have or claim to have against Studio Dog.

 

You acknowledge and agree whenever you request to book an appointment, that all the information you provided in this Agreement is accurate and complete, to the absolute best of your knowledge and that you have read, understand and accepted the terms stated above. You further acknowledge ownership of or accepting guardianship and thereby consenting to financial responsibility for the pet for which Services will be provided.

 

Studio Dog holds the highest standards to ensure that your pet is handled with respect and cared for properly. We recognize and respect that all pets are living beings who have feelings and experience emotions. We value that you have entrusted us to provide our services to them.
